~1) Name - shipping address and phone (I will call this # on Tues to get cc info. for orders) or let me know if you want to send a check.

~2)Specifics - Dime or Nickel size disc. 16 or 18 inch chain

~3) Names for each disc and design stamp choice - heart, flower, star, circle swirl, dragonfly, crown, peace sign, cross and sports balls.

~4) Special requests - ie: hammered texture - Initial Disc with March birthstone and heart design disc. etc...

Imprints Run - $36 for the chain and one disc and $14 for each additional disc. Initial Discs and small design stamp discs run $10 ea. and birthstones are $3 ea. (cording for Initial discs run $5 ea. colors depend on what I have in stock)
